New Massage Consultation Form for Texas Massage Therapists

The Texas Department of Licensing and Regulation (TDLR) released a revised Massage Consultation Document for massage therapists. The Texas Administrative Code requires massage therapists to provide a consultation document under the following circumstances:

  • Before a client’s first massage therapy session
  • If the client’s reason for seeking massage therapy changes
  • If any information in 16 TAC, Section 117.91(a)(1-4) is modified
  • Before each session during which breast massage will be performed (unless written consent is received on a separate document)

Use this checklist on page two to make sure your current consultation document meets the current TDLR requirements. Page three is a sample consultation document for you to reference to create or update your own. As the use of a consultation document is a Texas rule, failure to comply could result in disciplinary action.
